Mission Statement:

The mission of the Lifespan Community Health Institute is to eliminate health disparities and promote health equity through healthy behaviors, healthy relationships, and healthy environments.

Goal of Partnership:

Our goal is to connect our patients connect to resources in their area to essentially keep them healthy. In FI 2020 Connect for Health served over 1300 patients presenting with over 4500 social needs including housing, job training, food, utilities, and commodities.

Connect for Health Advocate

Main Duties:

  1. Working with patients who call or walk up to the desk
  2. Conduct intake including obtaining demographic information and assessing patients’ social needs and providing them with in-person assistance on a variety of resources for varied social needs
  3. Entering provider referrals and screens into an online database, calling referred or screened patients and keeping up with a caseload of patients based on number of hours of service per week
  4. Attend weekly Advocate Development meetings and supervision meetings

  • Interested in public health, public service, social work, health equity, human services, and social justice dedicated to health equity
  • Organized, dedicated ability to work independently and in a team-based approach
  • High level of personal accountability for the quality and impact of work
  • Excellent communication skills, professional demeanor, sound judgement, and strong organizational skills
  • Attention to detail
  • Ability to multitask & willingness to learn

Expected Hours per Week

Student is expected to work a minimum of 7 hours per week.
